Records Request

If you are a study participant and would like to request your GuLFSTUDY records please contact the study hotline (1-855-644-4853). You may also make a request in writing by mailing it to the address below or emailing your request to us at info@gulfstudy.nih.gov. If you are mailing or emailing your request, please be specific about the information or tests you are requesting and provide your contact information. Upon receipt of your request, our study team will reach out to confirm your identity and participant status.

If you are an attorney requesting records on behalf of your client, please mail a request to the GuLFSTUDY office at the address below. Your request should include a letter explaining what you are requesting, and a notarized, HIPAA compliant medical request form signed by the relevant study participant.
